1.      Purchase request 44130 for $60,000.00 to Schutts & Bowen LP for employee and employment related legal services (Agenda Memos)

 

2.      Purchase request 44263 for $87,719.47 to Physio-Control, Inc. for the purchase of three LifePak 15 monitor/defibrillators and upgrading one (1) LifePak 12 monitor/defibrillator, requested by the Fire Department. (Agenda Memos)

 

3.      Purchase request 44228 for $28,000.00 to Preferred Governmental for workers compensation insurance administration fee, requested by the Human Resources Department. (Agenda Memos)

 

C.    RESOLUTIONS:

 

1.      Resolutions authorizing amendments to extend agreements for professional services to provide city engineering services on a continuing basis (Agenda Memos)

 

A.    A resolution of the City Commission of the City of Leesburg, Florida authorizing the Mayor and the City Clerk to execute an Assignment and Consent Agreement whereby AECOM Technical Services, Inc. becomes the Successor In Interest of an agreement for professional engineering services from AECOM USA; and providing an effective date. B.     Resolution of the City Commission of the City of Leesburg, Florida authorizing the Mayor and the City Clerk to execute Amendment One to the contract  with AECOM Technical Services, Inc., successor in interest to AECOM USA, INC., for the purposes of providing professional engineering services on a continuing basis, pursuant to resolution 8315; and providing an effective date. C.    A resolution of the City Commission of the City of Leesburg, Florida authorizing the Mayor and the City Clerk to execute amendment one extending an agreement for professional services with Jones Edmunds & Associates, Inc. for the purpose of providing engineering services on a continuing basis; and providing an effective date. 2.      Resolutions authorizing amendments to extend agreements for professional services to provide surveying services on a continuing basis (Agenda Memos)

 

A.    A resolution of the City Commission of the City of Leesburg, Florida authorizing the Mayor and the City Clerk to execute amendment one extending an agreement for professional services with Dyer, Riddle, Mills & Precourt, Inc. for the purpose of providing surveying services on a continuing basis; and providing an effective date. B.     Resolution of the City Commission of the City of Leesburg, Florida authorizing the Mayor and the City Clerk to execute amendment one extending an agreement for professional services with Southeastern Surveying & Mapping Corporation for the purpose of providing surveying services on a continuing basis, pursuant to Resolution 8335; and providing an effective date. 3.      A resolution of the City Commission of the City of Leesburg, Florida authorizing the Mayor and the City Clerk to execute a Roadway Illumination Services Agreement with the State of Florida Department of Transportation to provide highway lighting along SR 500 (US Hwy. 441) from Perkins Street to Griffin Road to SR 27 Griffin to Shelfer Street (Agenda Memos, Supporting Docs)

 

4.      Resolution of the City Commission of the City of Leesburg authorizing the Mayor and the City Clerk to allow the Police Department to accept a Firehouse Subs Public Safety Grant for five AED Defibrillator's (Agenda Memos, Supporting Docs)

This section is reserved for members of the public to bring up matters of concern or opportunities for praise.  Issues brought up will not be discussed in detail at this meeting.  Issues will either be referred to the proper staff or will be scheduled for consideration at a future City Commission Meeting.  Comments are limited to two minutes.
